HARMONY Center for Surgery of the Ears, Nose and Throat has developed a patient privacy policy in accordance with the Health Insurance Portability and Accountability Act (HIPAA). Given that this practice submits insurance claims electronically, this law and the implementing regulations govern all personally identifiable health information about the patient, regardless of the form of information (e.g., electronic, oral, written, etc.)

HARMONY Center for Surgery of the Ears, Nose and Throat will act as steward of personally identifiable health information, and treat all medical record information – and its related financial, demographic, and lifestyle information as confidential.

• All patient identifiers will be removed prior to statistical reporting and research studies.

• Electronic mail, claims, eligibility, or referral information will not be transmitted without a patient’s written consent.
